Medicaid

  • We are and have been a Medicaid Sooner Start provider since the beginning. Medicaid is dependent upon authorization that is dependent on the submission of an evaluation or re-evaluation, and physician's order on the HCFA 12A form and a current IEP (when applicable). All children who are in school and have an individual education plan (IEP) must have this information submitted at the time when all the other authorization paperwork is submitted or they will be denied services. So please be timely in bringing this item to the office when requested.

  • Community Care (We have been trying since January 2009. All paperwork has been submitted and we are awaiting a reply)

Out-of-network insurance companies for Psychology and OT

  • Private company-funded programs
  • We will file with any insurance company and depending on the policy, many will pay for our services as out-of-network providers
